> CBD can alter exposure to some medicines, and other medicines can affect CBD exposure. A pharmacist or prescriber needs the exact CBD formula and full medication list to assess a particular combination.

## Why interactions matter

CBD can affect the way other medicines work, and other medicines can affect CBD exposure. FDA notes that a combination can increase adverse effects or reduce a medicine’s effectiveness. [1]

## Bring a complete list

Include prescriptions, over-the-counter medicines, supplements, herbal products, alcohol use, and any other cannabis products. Bring the CBD label and lab report rather than only its brand name.

## Ask practical questions

- Could this change how one of my medicines works?
- Could the combination increase sleepiness?
- Does my medical history change the risk?
- Would monitoring be needed?

Write down the answers so you can refer to them later.

## Do not adjust a prescription yourself

Spacing products a few hours apart is not a reliable general solution to metabolic interactions. Do not stop, reduce, or replace a prescribed medicine to make room for CBD. A pharmacist or prescriber needs to assess the specific combination.

## Turn an interaction headline into a precise question

A medicine’s name on a warning list does not describe the size, direction or clinical importance of every possible interaction.

| Evidence type | Useful detail | Question it leaves |
| --- | --- | --- |
| Human enzyme-probe experiment | A CBD–THC extract changed exposure to several probe drugs. | Does the exact medicine and formulation match? |
| Controlled clobazam trial | Exposure to the active N-desmethylclobazam metabolite increased. | Which measured substance and clinical context matter? |
| Laboratory or computer prediction | Identifies a possible mechanism to investigate. | Has it been demonstrated in people? |
| My medication list | Contains actual medicines, supplements and product ingredients. | What does a pharmacist conclude for this combination? |

An [18-adult crossover study](https://pubmed.ncbi.nlm.nih.gov/37313955/) measured drug exposure after a CBD–THC extract. A [clobazam trial](https://pubmed.ncbi.nlm.nih.gov/32652616/) found increased exposure to an active metabolite. The exact medicine and measured substance matter when interpreting either result.

For example, a hypothetical doubling of metabolite exposure would not mean the prescribed dose or symptoms doubled. Nor would it establish the same change for every medicine in that class. Show the pharmacist the actual medicine, CBD product and complete ingredients. These studies do not justify changing a prescription or assuming that separating products by a few hours prevents an interaction.

1. **Exact ingredients:** Medicine, supplement and CBD formulation.
2. **Exact finding:** Measured exposure, metabolite or prediction.
3. **Specific assessment:** Ask whether that evidence applies here.

Make one combined medicines-and-products list and ask which interaction evidence applies to the exact ingredients you have recorded.

Scope: These studies do not assess every medicine, retail formulation or personal medical history.

## Does a metabolite increase mean my prescribed dose has increased?

No. It describes exposure to a substance formed by metabolism. A clinician must interpret its relevance to treatment.

## Can a grapefruit-warning list clear medicines not listed on it?

No. Absence from such a list is not a complete interaction assessment for CBD or a finished product.

## What to bring to a medicines review

Include the exact CBD formula, prescribed and nonprescription medicines, supplements and other active ingredients.
